Job Responsibilities:

• Contact person: in the patient: inside from admission to discharge or transfer to other care systems
• Advice on long-term care insurance, including application, the transition to the home area and thus networking with outpatient care services, outpatient hospice services, networks such as SAPV and other therapists and aftercare providers, the initiation of inpatient care in nursing homes, residential groups or hospices, as well as the initiation of rehabilitation measures (AHB ) and early neurological rehabilitation
• The focus of the work will be advising and care planning for oncological patients in the cancer centers
• Post-clinical care is planned, coordinated, documented and evaluated according to the individual needs of the patient and the available resources of the clinic
• Recognizing the need for aids, advice on powers of attorney and living wills, contact with payers, authorities, rehabilitation clinics, self-help groups, advice centers, inpatient facilities and outpatient aftercare providers as well as in-hospital cooperation with the employees of the nursing service, the medical service and therapists from other professional groups

Job Requirements:

• Excellent communication and cooperation skills with all parties involved as well as a high degree of reliability, flexibility and resilience
• Strong organizational and analytical skills, above-average commitment and a result- and goal-oriented way of working
• Completed degree as a social worker (m/f/d) and professional experience as a social worker (m/f/d) in the healthcare sector, ideally in a hospital or in a rehabilitation clinic
• Comprehensive knowledge of the supply structures and offers of help and a competent and responsible ability to assess and select
• A high level of social competence and sensitivity in dealing with patients and their relatives in the sense of the Helios service concept and the quality of care as well as a high level of consulting competence and process-oriented work
• Distinctively quick case processing and very high resilience due to the shortened length of stay of the patients
